$1,450 CREMATION
As we know, funeral cost are on the rise, but salaries are staying the same. Also, all be it unfortunate, many people still die without life insurance leaving their family and loved ones to worry about burial cost. Simply notify us that the death has occurred, pay the required fees, and a representative will meet you at the hospital, nursing home, your home, in our office, or wherever you need us to meet you. Once we meet with you, we will complete and notarize all of the necessary cremation paperwork, contact the crematorium, and report the death to the proper government agencies.

You can trust that the crematorium will ID and properly dispose of your loved ones remains. The crematorium that we are in alliance with has an 12 step identification process: "1.) A wrist band identification is placed on the deceased; and if family is present, they are invited to write the deceased's name on tag. 2.) The deceased is then placed into a cremation container (law in Illinois) and the person's name is written directly on the container. 3.) Family signs an authorization for cremation, verifying the death of their loved one and giving us permission in writing to do the cremation. 4.) Copies of these documents are attached to the outside of the cremation container. 5.) The county medical examiner or coroner acknowledges awareness of the death and verifies cause of death or requires an autopsy or investigation. 6.) The death is recorded with the county and a cremation permit is issued (the permit is also attached to the cremation container). 7.)The death certificate and medical examiner's permit are filed with the State. 8.) Certified copies of the death certificate are available to the family and a State permit is issued, giving us the authority to perform the cremation. Only now can the cremation be performed. 9.) Crematory operator fills out a crematory control sheet, which requires two personnel to sign off on prior to the cremation being performed. 10.)The cremation is logged in the record book. The name of the deceased, date, cremation chamber used, time started and operator's name are recorded. Copies of #3, 4, 5, and 6 are attached to the outside of the cremation chamber. 11.)Paperwork is prepared in the office while the cremation is taking place. Immediately following the completion of the cremation the cremated remains are placed in an urn with the deceased's name already on the container. 12.) The cremated remains are returned to the family. The family must show picture I.D. and sign a release stating they are taking custody of the cremated remains."

CANCELLATION POLICY
We understand your time is valuable, and so is ours. Out of respect for our staff and clients, we request you give us at least 48 hours’ notice should you need to cancel an appointment. A cancellation or request to reschedule made less than 24 hours in advance will invoke a cancellation fee. The amount charged will be $25.
Should you arrive more than 20 minutes late for your appointment, we cannot guarantee that we will provide the service. If we cannot accommodate you, the same cancellation fee will be charged. We will try our best to reschedule your appointment for another time that is available. A credit card is required to make an appointment. Cancellation fees will be charged to the card you provide. In case of a true, unforeseen, and unavoidable emergency, a cancellation fee will be charged, but part or full amount of it may be deducted from your future appointment cost.
True Emergencies:
We realize sometimes true emergencies occur. If you do need to cancel or reschedule your appointment, we simply request that you inform us at least 24 hours in advance. If the booking was made for the same day, we require at least 4 hours’ notice. Cancellations that do not meet the above requirements will result in a $25 fee charged to the credit card on file or added to your next booking.
REFUND POLICY
All sales are final, and all services/sales are NONREFUNDABLE. Services fees outlined herein are fully earned for services rendered and nonrefundable for any reason. We reserve the right to offer account credits at our discretion.
